Form Creator
Create fillable PDF forms: text boxes, checkboxes, radio buttons, dropdowns, lists, date fields, signature fields and buttons. Values and choices can be in Arabic.
About This Tool
Form Creator adds fillable fields to a PDF: text boxes, checkboxes, radio buttons, dropdowns, list boxes, date fields, signature fields and buttons. Start from your own PDF, from a blank page, or from one of six ready-made layouts such as a contact form or an invoice; on the Arabic site these are written in Arabic and laid out from the right.
Pick a field type from the "Add" list and click the page to place it; drag a field to move it and pull its corner to resize it. Each field can have a name, a label printed beside it, a default value and, for lists, its options, and all of these can be written in Arabic. A field can be marked as required. A date field keeps to the format you choose, a signature field is one that a signing program can sign, and a button prints the form or clears it.
All processing happens locally in your browser, ensuring your documents remain private.

Frequently Asked Questions
What field types can I add?
Text fields of one line or several, checkboxes, radio buttons, dropdowns, list boxes, date fields, signature fields and buttons. A button either prints the form or clears what was filled in; it cannot send the form anywhere.
Can I make fields required?
Yes: tick "Required field" under "Properties", and the field is marked as required in the PDF. What a PDF program does with that differs: most only show the field as required, and do not keep a form from being saved or printed with it left empty.
Can I add calculations?
No. Fields cannot calculate anything. A total has to be typed in by the person who fills in the form.
What does a date field do?
It is a text box that keeps to the format you choose for it. A PDF program that runs a form's own checks, as Adobe Reader does, refuses what is not a date and shows the date in that format. A program that does not run them treats it as a text box, and so does Form Filler on this site.
Can the signature field be signed?
It is an empty signature field, shown as a box with the label you give it: a PDF program that signs with a certificate can put its signature there. Sign PDF on this site draws a signature on the page instead, and does not use the field.
